Primary care is the day-to-day healthcare given by a health care provider. Typically, this provider acts as the first contact and principal point of continuing care for patients within a healthcare system, and coordinates other specialist care that the patient may need. Patients commonly receive primary care from a medical profession such as a physician (internist, pediatrician) or a nurse practitioner.

Pediatrics is the branch of medicine dealing with the health and medical care of infants, children, and adolescents from birth usually to the age of 21.

Gynecology treats the overall health of female patients including preventative care and treating problems and diseases of the female reproductive system such as breast and hormonal problems, urinary tract and pelvic disorders, and cancer of the cervix.

Dentists provide treatment and care of diseases and other conditions that affect the teeth and gums and oral health for the entire family. This includes the diagnosis, prevention, and treatment as well as the removal, correction and replacement such as fillings, crowns, extractions, straightening of teeth and fitting of dentures.

Psychiatry is the branch of medicine focused on the diagnosis, treatment and prevention of mental, emotional and behavioral disorders.

Psychological tests provide a way to measure different factors that can contribute to people’s problems. A person is usually interviewed before a test is administered. A psychological test is “an objective and standardized measure of a sample of behavior”.

Physical therapists can teach patients how to prevent or manage their condition so that they will achieve long-term health benefits. Physical therapists examine each individual and develop a plan, using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability.

Podiatrists are responsible for the assessment, diagnosis and management of conditions of the foot, ankle and lower leg.

Our Nutritionists will advise on what to eat in order to lead a healthy lifestyle or achieve a specific health-related goal.

Ophthalmologists are physicians who specialize in the anatomy, functions and diseases of the eye. HCC’s Board certified physicians will provide medical and surgical eye disease prevention, care and treatment in vision care to our patients.

Smoking cessation or quitting smoking is the process of discontinuing tobacco smoking. Tobacco smoke contains nicotine, which is addictive. Nicotine withdrawal makes the process of quitting often very prolonged and difficult.  Our providers will work with you to find the best method to help you stop smoking/vaping.

Behavioral health includes not only ways of promoting well-being by preventing or intervening in mental illness such as depression or anxiety, but also has and can assist in preventing or intervening in substance abuse or other addictions.

BUPRENOPHINE TREATMENT is indicated for treatment of opioid dependence and should be used as part of a complete treatment plan to include counseling and psychosocial support. It is intended to help people overcome opiate addiction and maintain a commitment to recovery.

Buprenorphine is a medical prescription that might help you:

  • Stay physically comfortable while you’re in the early stages of recovery
  • Block your craving for heroin or other street opioids
  • Minimize your chances of having a relapse
  • Help you gradually and safely reduce your dependence on street drugs

Naloxone is an easy-to-use, lifesaving antidote to reverse overdose from heroin or other opioids.  We have certified trainers on staff that trains individuals or organizations on the use of Naloxone and distributes the intranasal kits to everyone who completes the training.  This approach has been very successful in reaching the community to offer a tool to save the lives of themselves, either family, friends or the general community.

HealthCare Choices offers SAME-DAY appointments for primary medical care for children and adults. If you or a family member are not feeling well, HCC will tell you to come in on that day to see your primary care provider.

A well visit (or check-up) is a scheduled appointment with your doctor when you are not sick. This visit helps to prevent health problems and is a perfect chance for you to talk to your doctor or nurse.

Our Care management program is a free program for people on Medicaid who struggle with chronic medical and/or mental health conditions. Our team of dedicated care managers help you get the care and services you need to stay healthy.
